David Gellett, 30, of Bollam Close, Connah's Quay, a goalkeeper for Wepre Rangers Football Club, collapsed before a match against Holywell Town at their ground in Halkyn Road in July.
Despite frantic efforts to resuscitate him, Mr Gellett was pronounced dead at Glan Clwyd Hospital, Bodelwyddan, less than an hour later.
In the days after his death tributes flooded into the Leader's website, and his sister, Nicola Derbyshire Gellett, spoke of her devastation at the loss of her much-loved brother.
"He was larger than life and I was very, very proud of him," she said at the time.
"We were very close and he always looked after me.
"His mates were the world to him, he had really great friends."
Mr Gellett worked at Challenger Mobile Communications, Deeside, and his colleagues have recently set up a memorial plaque for him in the company building's entrance hall.
Carly Burrows, who worked alongside Mr Gellett, said: "We had a collection for Dave just after his death and we used most of the money on flowers for his funeral.
"We had some left over, so we spoke to his dad and sister and came up with the idea of a memorial plaque. They seemed very touched by the idea.
"We wanted to make sure that nobody forgets Dave and the plaque is in a place where everyone can see it on their way in.
"We have customers who knew Dave too, so they will also be able to look at it and remember him."
The plaque, which has a picture of Mr Gellett and is mounted on the wall, reads: "Forever blowing bubbles" – a tribute to his lifelong support of West Ham United Football Club.
Carly added: "Dave was a really lovely person and everyone knew and liked him.
"It was absolutely devastating for us to hear of his death, because he had been in work and was absolutely fine the day before. It was a huge shock.
"It was so sudden and I cannot imagine how his family must have felt.
"No-one had a chance to say goodbye to Dave, but hopefully this plaque will be a memory of him that can last forever."
